The Service of Remembrance will be held at 3 p.m. on Sunday, Feb. 19 at First Presbyterian Church at 918 N. Main St. in High Point for those who have experienced the death of a loved one in 2011.

Reaching Out: Ways to Help Others Experiencing a Serious, Life-limiting Illness

Care Connection, a division of Hospice of the Piedmont, provides home health care for individuals with life-threatening illness who need skilled care at home. For admission to the program, a patient of any age must be homebound and have a life-limiting illness that requires skilled nursing care. Some patients in Care Connection may be pursing aggressive, curative treatment; others are focusing on comfort measures only. Special emphasis is placed on the transition patients make when being discharged from a hospital. Care Connection is a highly cost-effective alternative to hospitalization, and reimbursement for care is received from Medicare, Medicaid, and private insurance companies.
