Team Leader

This position coordinates the delivery of quality interdisciplinary care for the patients and families in the Hospice and Care Connection programs and acts as a resource and back-up to the primary nursing staff to ensure continuity of care. Candidate must possess an ASN from an accredited school of nursing (BSN preferred), a minimum of 5 years experience in a supervision role, current NC RN license, current NC drivers license, ability to maintain a positive relationship with co-workers and the public, excellent oral and written communication skills and be computer proficient.
Staff Development and Performance Improvement Coordinator

This position directs the agency Performance Improvement Program to ensure compliance with regulatory requirements and standards of care and coordinates the education program to ensure staff growth and quality of care. Candidate must possess a BSN from an accredited school of nursing, strong analytical and interpersonal skills, intermediate to advanced level computer skills, an ability to train others and a minimum of 5 years experience in a similar role. A strong knowledge of hospice and home health regulations is a plus.
Homecare Staff RN (Full-time)

Requirements: Associates Degree (BSN preferred) from an accredited school of nursing and a minimum of 2 years clinical nursing experience; possess a current RN license and valid NC drivers license.
Major Duties and Responsibilities: Performs hospice and palliative care to patients in homecare setting. Assesses physical, environmental/safety needs of the patient. Develops and coordinates plan of care and makes visits according to such plan.
